Surgical Release

Registration Emergency Medical Form

All students are required to fill out the following forms, Physicians Referral form, Emergency Medical form, Liability Release form, and News and Photo Release form. If the student has had recent surgery the Surgical Release form must be filled out. If the student has a therapist the Therapist Release form must be filled out. And if the student is in school the Teacher evaluation form must also be filled out.

All FORMS are written in Microsoft Word and may be filled in on the computer and printed out.  Forms must have original signatures and mailed to the office by March 15, 2008
