Registration Emergency Medical Form

All students are required to fill out the following forms, Physicians Referral form, Emergency Medical form, Liability Release form, and News and Photo Release form. If the student has had recent surgery the Surgical Release form must be filled out. If the student has a therapist the Therapist Assessment form must be filled out. And if the student is in school the Teacher evaluation form must also be filled out. All these forms must be filled out once a year.

ALL STUDENTS MUST BE FREDERICK COUNTY MD RESIDENTS

All FORMS are written in Microsoft Word and may be filled in on the computer and printed out. Forms must have original signatures and mailed to the office by March 15 for the spring session, or August 15 for the fall session.

Waiting List Application Form

Please complete the WAITING LIST APPLICATION and return to the office. Once we receive your application form, you will be placed on the WAITING LIST. When you reach the top of our WAITING LIST, you will be asked to complete the remainder of the forms, including the Physicians Referral Form.

Individuals come off of the WAITING LIST when one of our Current Riders decides to no longer participate. We currently have 76 rider positions. Once you become a Current Rider with Frederick County 4H Therapeutic Riding Program, you may continue riding as long as you like, simply by updating your forms annually.
